The Mississippi State Department of Health (MSDH) has asked us to share this survey with you.
The goal is to use the answers we give them to come up with the best strategies for slowing down the spread of COVID-19 in our communities. The survey is only for Mississippi residents.
MSDH has worked with Jackson State University’s Mississippi Urban Research Center to create an online survey that you can take from a computer or mobile device.
