Our partner, the Children and Youth with Special Health Care Needs Program at the Mississippi State Department of Health, shared this funding announcement:
“The Mississippi State Department of Health (MSDH), Children and Youth with Special Health Care Needs (CYSHCN) Program announces the availability of funds for the CYSHCN Cares 2, a care-coordination learning collaborative. The purpose of this award is to optimize the quality of life for CYSHCN by aligning medical/dental homes and community support services.
Awardees will be granted $100,000 which covers the period of August 15, 2020, through June 30, 2021. Please complete the attached application and return by mail or fax on or before July 30, 2020. Attached is a Word version of the application for your convenience. Awarded healthcare systems will be notified on August 6, 2020.
